Two ACT workers were recently showered with concrete after entering an unsafe zone where concrete was being discharged. The area was not identified with bunting nor were other workers tasked to protect the discharge zone, ACT WorkCover notes in the attached alert.

Mechanical failure in the jib during commissioning caused the boom structure of a tower crane to come crashing down on BHP’s Worsley Alumina Refinery project near Collie, WA, on Wednesday afternoon January 13.

The boom damaged a number of amenities sheds and another crane, and narrowly missed some workers, but fortunately no-one was injured.

Major contractor John Holland Pty Ltd has been fined $180,000 over safety failures that resulted in the death of Mark McCallum at Dalrymple Bay Coal Terminal in May 2008.

The 34-year-old crane operator and rigger was working on a coal loading jetty when he was crushed by a moving jinker.

The court found the work method put Mark McCallum out of the range of vision of operator of the front-end loader and there was no radio protocol to confirm receipt of instructions.

Lampson (Australia) Pty Ltd is a crane company specialising in large crawler cranes. Their yard is based in a busy industrial estate in Bassendean. Perth. Around 3:00pm on Monday 11th January 2010, the CFMEU WA Branch was notified of an incident where a crane had fallen over and took out powerlines.

Just one month after the serious injury of a worker in Perth from a scaffold stair collapse, there was another incident with scaffold stairs on the Hansen Yuncken Pty Ltd site at Lyell McEwin Hospital in north Adelaide on September 25.

Untold numbers of workers and passers-by might have been seriously injured in central Sydney on Monday November 9, were it not for the quick thinking of a crane operator and his dogger.

CFMEU ACT safety officers uncovered dangerous removal of asbestos from Pickles Auction House in Fyshwick on Friday October 9. WorkCover ACT has placed a prohibition notice the worksite due to the danger that it poses to the public.

It has also been revealed that the ACT Planning and Land Authority has ordered that the site be closed, as the company removing the asbestos did not seek approval for any redevelopment of the site.

Australasian Technical Services (ATS), the company involved in the exposure incident, subsequently had their asbestos removal license suspended.

A Victorian man was killed during a concrete pour on the old Pentridge Prison site at Coburg in Melbourne's north Thursday October 15.

Yet the builder tried to deny access to CFMEU officials attending the site to supervise the safe retrieval of the concrete pump the following day.

The ABCC also phoned the Union, more concerned about the Union presence on the job than the safety matters under investigation.
