Nylon Sling failure
A structural steel and bondeck roof section, complete with handrails and weighing approximately 11 tonne, fell to the ground in WA recently, after nylon slings failed. The roof section was being lifted by the tower crane to be positioned on top of a silo. 
Fortunately, no-one was injured or killed.
The incident is another reminder of the dangers when using nylon slings.
